Youth Transition
Transition ...
It is that time in a young person's life when they are preparing to leave high school and venture out into the adult world. This time can be challenging for any young adult, and even more challenging for a young adult with a disability. Students often ask themselves:
Where am I going to live?
What do I want to do for a living?
Students with disabilities faced with the transition to adult life may also ask themselves:
How am I going to reach my goals while dealing with the barriers I face because of my disability?
The North Country Center for Independence has been awarded a four-year grant through the New York State Office of Vocational and Educational Services for Individuals with Disabilities, (VESID), to assist Plattsburgh High School and Peru Central High School students with disabilities to plan for a successful transition to adult life. NCCI's Transition Services will have three main components:
One-On-One Services:
Students can meet individually with a Transition Counselor, by appointment or drop-in, at their own school. They can use this time to ask questions about the transition planning process, and to discuss their goals for adult life, while building their self-advocacy skills so that the students are at the center of their own transition plans.
Group Activities:
Learning and discussion in a classroom setting, where students can work as a team and learn independent living skills. For example: financial management, transportation, organizational techniques, job-seeking skills, social skills, cooking, etc.
Peer Mentoring:
Students have the opportunity to meet with adults who also have disabilities who act as volunteer Peer Mentors, sharing their experiences and giving advice on living a successful adult life with a disability. For more information on mentoring, visit our Mentoring page.
We are available to work individually with any young person thinking about the transition to adult life, as well as parents and educators. We can also address groups on Transition topics like:
Self-advocacy skills
Independent living skills
Continuing education
Employment
Community services and adaptive technologies
